Residence card in Poland: documents and guides
Choose your grounds for residence and open a checklist. The final set of documents depends on your circumstances and the immigration office’s current requirements.
Official MOS forms and requirements · Załącznik nr 1 & nr 2 — current templates · Załącznik nr 1 — video guide · Video: work-based residence card · Video: EU long-term resident status · Profil Zaufany · PDF / gov.pl · Explanations on YouTube
EU long-term resident • Valid passport and evidence of lawful continuous residence. • Evidence of stable, regular income for the required period. • Health insurance and a legal title to your accommodation. • Accepted evidence of Polish language proficiency at level B1 or above. • Application, current photo and payment confirmations following MOS instructions. • Video: 10 tips before applying for EU long-term resident status — linked at the top of this page. • Usually 5 years of residence and 3 years of income are assessed; exceptions and absences require individual review.
Temporary residence — employment • Valid passport: scans of every page, and a current digital photo. • Electronic MOS application and required payment confirmations. • Załącznik nr 1 completed and signed by an authorised employer representative. • Contract and evidence of employment conditions; insurance depending on the contract. • If relevant: qualifications, previous decisions and evidence that earlier employment ended. • Video walkthrough of employer and work-agency documents — linked at the top of this page. • The applicant personally signs and submits the application. Additional documents depend on the case.
Business — sole trader (JDG) • Passport, photo, application and required payment confirmations. • CEIDG/NIP details, client contracts and evidence of business activity. • Accounting records and tax return with proof of submission. • ZUS records, health insurance and evidence of accommodation. • Business financial results; where relevant, employee records, business plan, investment and development funds. • A limited company (Sp. z o.o.) needs a separate checklist. Registering a business alone does not establish eligibility for residence.
Family grounds • Passport, photo, application and required payment confirmations. • Marriage or birth certificate and required sworn translations. • Passport and residence-status documents of the family member in Poland. • Evidence of income, insurance and accommodation, depending on the legal grounds. • For a child: parents’ documents and any additional case-specific evidence. • Family of a Blue Card holder: applications can be filed together with the main applicant, with proof of their permit or MOS filing. Children aged 6+ give fingerprints. • Family reunification and other family grounds have different conditions. We first identify the appropriate procedure.
EU Blue Card — highly qualified work • Passport, digital photo, MOS application and required payment confirmations. • Electronic Załącznik nr 1 from the employer; Załącznik nr 2 information forms part of the application. • Both Załączniki are completed online in MOS; current templates and a video guide are linked at the top of this page. • Contract with employment conditions meeting Blue Card requirements. • Salary under the contract of at least 150% of the Polish average wage: PLN 13,355.34 gross per month for applications filed in 2026. • Evidence of higher qualifications: diploma or qualifying professional experience. • Required translations and health-insurance documents. • Contract duration, salary and qualification evidence must meet the current requirements.
Permanent residence — Karta Polaka • Valid passport, application, photo and documents under current MOS instructions. • Copy of a valid Karta Polaka; original available for inspection. • Declaration of intent to settle permanently in Poland. • Evidence supporting this intention, appropriate to your circumstances. • Required translations and any further documents requested by the office. • This checklist applies to Karta Polaka holders, not to every permanent-residence route.
Permanent residence — Polish origin • Passport, application, photo and payment confirmations under MOS instructions. • Documents proving Polish origin and the family relationship. • Civil-status records and required sworn translations. • Evidence of connections with Polish culture and traditions. • Declaration and evidence of intent to settle permanently in Poland. • We assess the evidence of origin and family links individually before preparing an application.
